Essential Kidney Function Test Parameters You Should Know

Kidney function parameters provide critical information regarding the kidneys’ ability to filter blood, eliminate waste products, and detect potential renal abnormalities. Key indicators commonly evaluated include Serum Creatinine, Urea/BUN, eGFR, Cystatin C, and various urine markers.

Each parameter reflects a different aspect of renal health. Therefore, results must be evaluated comprehensively rather than making self-diagnoses based on a single abnormal figure.

Key Blood Indicators for Kidney Function

Serum Creatinine

Creatinine is a breakdown product of creatine phosphate from muscle metabolism, which is filtered by the kidneys and excreted in urine. Serum creatinine is one of the most widely used markers to assess kidney filtration capacity.

Normal Reference Ranges:

  • Men: 0.7 - 1.3 mg/dL (equivalent to 62 - 115 µmol/L)
  • Women: 0.5 - 1.1 mg/dL (equivalent to44 - 97 µmol/L)

Creatinine levels can be influenced by muscle mass, age, diet, and other physiological factors. Consequently, an elevated creatinine level does not automatically indicate renal impairment and must be evaluated alongside other clinical data.

Urea / Blood Urea Nitrogen (BUN)

Urea is a byproduct of protein breakdown, primarily excreted by the kidneys. BUN measures the amount of urea nitrogen present in the blood.

Normal Reference Ranges:

  • Blood Urea: 2.5 - 7.5 mmol/L (equivalent to 15 - 45 mg/dL)
  • BUN: 6 - 20 mg/dL (equivalent to 2.1 - 7.1 mmol/L)

Urea/BUN levels provide complementary insight into renal function. However, values can fluctuate due to dehydration, high-protein diets, specific medical conditions, or medications.

As a supporting marker, it should not be used in isolation to diagnose kidney health.

eGFR / GFR (Estimated Glomerular Filtration Rate)

GFR reflects the volume of blood filtered by the glomeruli per unit of time. In clinical practice, eGFR is calculated using equations based on serum creatinine, age, and biological sex.

Standard Unit of Measurement: mL/min/1.73 m²

eGFR Classification Stages:

  • eGFR ≥ 90: Normal renal function (Stage G1)
  • eGFR 60 – 89: Mildly decreased (Stage G2)
  • eGFR 30 – 59: Moderately to severely decreased (Stage G3a – G3b)
  • eGFR 15 – 29: Severely decreased (Stage G4)
  • eGFR<15: End-Stage Renal Disease / Kidney Failure (Stage G5)

Key functions of eGFR include:

  • Assessing the overall filtering efficiency of the kidneys.
  • Monitoring changes in renal function over time.
  • Staging the severity of chronic kidney disease when combined with relevant clinical data.

Because eGFR is an estimated value, it must always be interpreted in the context of patient age, underlying conditions, and related lab findings.

Cystatin C

Cystatin C is a low-molecular-weight protein produced at a constant rate by all nucleated cells, serving as a reliable surrogate marker for glomerular filtration.

Normal Reference Range: 0.6 - 1.0 mg/L

When serum creatinine accuracy is compromised by low muscle mass or other confounding factors, Cystatin C testing can yield clearer information regarding renal filtration performance.

Its use depends on clinical goals and patient context.

Key Urine Parameters for Kidney Assessment

In addition to blood tests, urine parameters offer essential diagnostic information when evaluating kidney condition.

Urinalysis markers for kidney function

Proteinuria (Urine Protein)

Abnormal levels of protein in the urine can indicate glomerular filtration barrier damage or renal disease.

Standard Reference Ranges:

  • Qualitative (Dipstick Test): Negative (-)
  • 24-Hour Urine Protein Quantification: < 150 mg/24 giờ
  • Urine Protein-to-Creatinine Ratio (UPCR): < 150 - 200 mg/g

Proteinuria can occur transiently due to fever, strenuous exercise, or dehydration. Finding protein in a single urine specimen does not confirm chronic kidney disease.

Depending on the case, quantitative testing for urine protein or albumin may be required.

Urinalysis (Routine Urine Test)

A standard urinalysis measures various key components, including:

  • Protein
  • Blood / Red Blood Cells (RBCs)
  • White Blood Cells (WBCs)
  • Glucose
  • Other abnormal components (Bilirubin, Ketones, Nitrites, etc.)

While urinalysis helps identify abnormalities involving the kidneys and urinary tract, not all abnormal findings stem from primary renal disease.

Evaluating Kidney Function Requires a Holistic View

No single kidney parameter can provide a complete picture of renal health. Results must be interpreted collectively rather than individually.

When reviewing your results, physicians evaluate:

  • Degree of Abnormality: How much a value deviates from standard laboratory reference ranges.
  • Inter-parameter Correlation: Whether blood and urine values show concurrent abnormalities.
  • Trends Over Time: How current results compare to previous diagnostic tests.
  • Overall Health Context: How age, existing conditions, current medications, and lifestyle factors influence test values.

  • Your eGFR has declined compared to previous tests.
  • Protein or blood is detected in your urine.
  • You are being monitored for kidney disease or related chronic conditions.
  • You need clear medical guidance interpreting your lab report.
